2009 Whammy IV. I have replaced the missing screws in the pictures. Not sure why I ended up with this pedal. I'm not a shredder so I basically helped a kid out and bought it about six weeks ago. I have not tested the MIDI part of the pedal.I'll cover shipping on the 48 states. I ship a day after purchase unless it's on the weekend. Thanks.
