Electro-Harmonix Deluxe Electric Mistress. Works great. 120v version. Includes pedal, box (cardboard, not wooden), warranty card, instructions, and Readers Digest reprint about Mike Matthews. Pedal has some surface rusting, but is in sturdy physical condition. It's hard to date this pedal exactly, but here is any relevant information I could come up with:The date code on the CTS pots reads R1378237. This should put the manufacture date of -that potentiometer- around Sep. 13, 1982. It appears to be original.The PCB has the code EH5150-D printed on it, which normally was used with the inverse green and black enclosures made ~1981.The reader's digest article included is from 1982.
